(must include additional color copy of information page/picture page of the passport)
1.2 photo requirement (2 identical, color, passport style photographs)
1.3 minor children under age 18 (kevin king family only)
1) legible copy of state issued birth certificate of child (get notarized)
2) color copy of parents passport & drivers license (get notarized)
